The goal of StaffMed Health Partners Referral Bonus Program (“Program”) is to receive referrals (“Referral”) of physicians and clinicians (“Candidate(s)”). In return, StaffMed Health Partners will pay a referral bonus (“Bonus”) to the referring party (“Referring Party”) based on the Candidate’s position, as outlined below, once the terms and conditions herein are met.
To qualify for a Bonus, the Candidate referred by the Referring Party must not have been presented by StaffMed Health Partners to any facility for any position within the past 12 months. Candidates who have been presented within the last 12 months will be considered active and ineligible for a Bonus.
(a) Qualifying Assignment: The Referring Party will earn a Bonus for each Referral made, provided the Candidate starts their assignment with StaffMed Health Partners and meets all contract requirements during those days worked (“Qualifying Assignment”).
(b) By referring a Candidate, the Referring Party certifies that the Referral and receipt of any Bonus for such Referral does not violate any applicable employment obligations to which the Referring Party is subject.
(c) Corporate employees of StaffMed Health Partners and its affiliates are not eligible to participate in this Program.
(a) The Referring Party will be credited for Referrals if:
i. The Referring Party completes the online referral form before the Candidate is presented by StaffMed Health Partners to a healthcare facility, and the Candidate confirms the Referral from the Referring Party. The Candidate must be placed with a StaffMed Health Partners client within 18 months of their initial referral by the Referring Party.
ii. Prior to being presented by StaffMed Health Partners to a healthcare facility, the Candidate informs StaffMed Health Partners that the Referring Party made the Referral between the Candidate and StaffMed Health Partners. The Candidate must be placed with a StaffMed Health Partners client within 18 months of their initial referral by the Referring Party.
(b) In the event a Candidate has been referred by multiple Referring Parties, the Candidate must determine in writing the sole Referring Party to whom a Bonus is paid.
(c) Once a Candidate’s Referring Party is confirmed, the Candidate is ineligible for a Referral by a different Referring Party.
(a) To receive a Bonus, the Referring Party must provide StaffMed Health Partners with a valid W-9 and current contact information within 30 days of the completion of the Candidate’s Qualifying Assignment.
(b) Bonus requests will be submitted for processing within 60 days of the Candidate’s 90th day/720th hour of work.
(c) All applicable federal, state, and local taxes are the Referring Party’s responsibility, in accordance with all applicable laws and regulations. The value of the Bonus will be reported on the applicable IRS form (1099-MISC).
(d) Bonuses may be paid via EFT or check, at StaffMed Health Partners’ discretion.
(e) In the case of “reciprocal introductions” where a Referring Party and a Candidate serve dual roles, each Referring Party will receive a one-half Bonus for a total payment equivalent to both Bonuses.
(f) The Bonus amount for any referral is $1,000.
(a) StaffMed Health Partners reserves the right to disqualify a Referring Party for any reason and at its absolute and sole discretion, including violations of these Terms and Conditions or for fraud.
(b) To the fullest extent permitted by law, neither StaffMed Health Partners nor its affiliates will be liable for damages arising out of or in connection with this offer.
(c) By referring a Candidate to StaffMed Health Partners, the Referring
Party certifies that they are NOT currently a government employee or active military.
(d) There is no limit to the number of Referrals a Referring Party can make or Bonuses a Referring Party can earn, subject to meeting all eligibility requirements, including but not limited to the Candidates’ completion of Qualifying Assignments.
(e) When providing a Referral, no stipulations, conditions, or requests as to where a Candidate may or may not work are permitted. If a stipulation, condition, or request is made in a Referral, StaffMed Health Partners reserves the right to refuse any such request.
(f) StaffMed Health Partners reserves the right to change the Program Terms and Conditions and/or end the Program at any time without notice.
The acronym “GOLD,” which stands for Gratitude, Ownership, Leadership, and Discipline.
